Hospital Discharge Pilot Program
Meals on Wheels NC has been awarded a contract with NC DAAS to administer this pilot program through September 2024. As a member of Meals on Wheels NC, you are eligible to take part in the Hospital Discharge Pilot Program with NC DAAS.
You could receive referrals for a 14 meal delivery for people who are recently discharged from the hospital and at risk of malnutrition, if you are registered on NC Cares 360. Your program would receive $15 per meal delivered.

Meal Provider Guidance
- Be able and willing to deliver meals immediately to the client. You do NOT need to add the client into ARMS as this payment is not associated with HCCBG. Meals MUST begin delivery 5 to 7 days after discharge
- Be willing to deliver a HOT meal DAILY for 2 weeks—this is an important requirement set by DAAS (if you can only provide frozen meals, provider must note why on the case file before closing)
- Be a member in good standing of Meals on Wheels NC
- Invoice Meals on Wheels NC each month you deliver meals for the pilot program
- Add a survey with self addressed envelope (provided by MOWNC) with the initial meal delivery for the client to fill out and send back
- After 30 days fill out a program survey for the clients you served. Emailed survey form sent you by MOWNC
The surveys are also a very important requirement set by DAAS
Not all programs will receive referrals, but you MUST be registered on NC CARES 360 to receive referrals.
